How do Lottery clubs claim prizes?

A lottery club is a group of at least two individuals who purchase wagers in a lottery game together.  If the club wins a prize, it is divided among the club members. It is up to the lottery club to determine its rules of operation, including the distribution of prize money to members.

Lottery clubs must submit a completed Substitute 5754 form to claim a prize. Click on the links below for instructions on how to form a club, the Substitute 5754 form, and instructions for completing that form.

If you have any questions, please feel free to call the Lottery's Public Relations Division at (517) 373-1237.
